The difference between a Tower and a Desktop PC is in it's design. Desktops are flat and rectangular and normally sit on the desktop, hence the name. Towers are more of an upright design that usually sits on the floor or a low shelf, maybe even underneah the desk. Your choice should be governed by the space you have available. There seems to be more of a preference for Towers these days, particularly since the introduction of the more compact mini-tower.
Neither a desktop PC or Tower PC normally includes the screen, keyboard, mouse or other peripherals. Tower or desktop usually only refers to the box containing the motherboard, the hard disk, cd and/or dvd drives, memory, graphics cards etc.
